It's just a little form you got to fill out.
Holder ID - User ID of who you want to hold a banner (if you're doing it for yourself, use your own id).
Media Asset ID - Upload whatever GIF or image you want (as long as the resolution is between 330px X 70px and 1320px X 280px) and then use the id from it. There's a direct link to your own assets and all assets available for easy access.
Destination URL - Link to where you want the banner to lead to. Must start with the "https" portion.
On-Hover Text - What you want the hover dialog box to say.
CSS Styling - You can either leave that be for the same background art as banner #2, or use ColorBeta to design your own banner background (if on PC, shrink the window vertically sometimes when designing it, or else the result will end up as extremely squished.
Priority - Low is the free banner that appears 40% of the time, High is the paid banner that appears 60% of the time, Retired removes the banner from the drawing pool without deleting it.
Gift Banner? - Set to "Yes" if you give users High Priority banners for free to make it easier for other admins to keep track of what requires payment or not.
